Oleander and Foxglove Kill Pets Within Hours — The Cardiac-Glycoside Plants You Need to Remove

Oleander, foxglove, and lily of the valley share one kill mechanism: they block the pump that keeps your pet’s heart beating. Here’s what to remove and what to plant instead.

A 2.4-kilogram Maltese chewed a few oleander leaves. Within 24 hours she was in a referral hospital with a second-degree AV block, a heart rate of 80 beats per minute, and hind-limb ataxia. She survived after 48 hours of intensive care — but the case report that documented her recovery also noted something that most gardening guides skip entirely: the mechanism that nearly killed her is identical to the one inside every foxglove and lily of the valley in your yard.

These three plants don’t just “contain toxins.” They carry cardiac glycosides — compounds that disable a specific molecular pump your pet’s heart cells cannot survive without. Understanding the shared mechanism changes how urgently you treat exposure, and why “my dog seems fine now” is exactly the wrong reason to skip the vet.

The Shared Kill Switch: Why These Three Plants Are a Category, Not a List

Every cell in your pet’s heart runs on a protein called the sodium-potassium ATPase pump (Na⁺/K⁺-ATPase). Its job is relentless and simple: push sodium out of the cell and pull potassium in, over and over, to maintain the electrical gradient that makes a heartbeat possible.

Cardiac glycosides lock onto this pump and freeze it in a disabled state. When Na⁺/K⁺-ATPase stops working, sodium accumulates inside the cardiac cell. The cell then tries to compensate by running a secondary exchanger — one that trades sodium for calcium. The result is a flood of intracellular calcium that triggers spontaneous electrical discharges, called afterdepolarizations, which corrupt the heart’s rhythm. The outcome ranges from slow, irregular pulses to ventricular arrhythmias that can be fatal without treatment [4, 5].

This mechanism is identical whether the glycoside is oleandrin from an oleander shrub, digitoxin from a foxglove spike, or convallarin from a lily of the valley ground cover. The three plants are botanical cousins in toxicology, not just a list of things to avoid. That shared pathway is also why the antidote — digoxin-specific antibody fragments (Fab), originally developed for human digoxin overdose — works across all three.

One more critical point before we cover each plant individually: cats appear to be more sensitive to cardiac glycoside plants than dogs [6]. The mechanism is the same, but cats’ smaller body mass and physiological differences mean that what causes arrhythmia in a larger dog can cause cardiac arrest in a cat. Treat any exposure in a cat as an emergency regardless of the amount observed.

Oleander (Nerium oleander): The Landscape Shrub With No Safe Dose

Oleander is a dense, drought-tolerant shrub planted widely across USDA Zones 8–11 for its showy clusters of pink, white, or red flowers. It’s a staple of Southern California medians, Florida landscaping, and Gulf Coast yards — which means it’s common exactly where dogs and cats spend the most time outdoors.

Every part of the plant is toxic: leaves, flowers, stems, roots, fruit, seed, nectar, and the water in any vase where cut oleander has stood [1, 9]. The primary toxins are oleandrin and digitoxigenin, both cardiac glycosides that inhibit Na⁺/K⁺-ATPase directly [5]. For cattle and horses, the lethal dose is as low as 0.005% of body weight — roughly one ounce of green leaves for a horse [8]. Specific lethal dose data for dogs and cats hasn’t been established in controlled studies, but clinical evidence makes clear that even small exposures — a few chewed leaves in a small dog — can produce life-threatening arrhythmias [7].

Onset is fast. The ASPCA lists drooling, abdominal pain, diarrhea, and colic as early signs, progressing to depression and death [1]. The MSD Veterinary Manual documents onset within two hours of ingestion, with signs potentially persisting for four to five days — meaning a dog who vomits once and appears to recover may be entering the cardiac phase [6]. A neighbor’s oleander overhanging a fence is enough exposure risk to warrant fencing off or removal.

Foxglove (Digitalis purpurea): The Cottage Garden Classic With a Hidden Payload

Foxglove is sold at every garden center for its tall spikes of tubular flowers in purple, pink, white, and cream. It naturalizes easily and self-seeds, which means removing one plant often means checking for seedlings the following spring. Like oleander, every part is toxic: leaves, flowers, stems, seeds, and roots. Even the water in a vase of cut foxglove can carry toxic concentrations of cardiac glycosides [10].

The dose risk is alarming relative to plant size. According to the German veterinary resource PetsVetCheck, as few as five dried foxglove leaves are considered potentially fatal for a medium-sized dog. Fresh leaves contain higher concentrations than dried, and seeds carry some of the highest glycoside loads. The ASPCA lists irregular heartbeats, vomiting, diarrhea, weakness, heart failure, and death as clinical outcomes for dogs and cats [2].

Foxglove is also a biennial — it grows as a rosette in year one with no flowers, then bolts and blooms in year two. The rosette stage is lower to the ground and looks more accessible to a curious dog nosing through garden beds. Lily of the Valley (Convallaria majalis): 38 Cardiac Toxins in Every Cluster

Lily of the valley is a beloved shade ground cover prized for its arching stems of small white bell flowers and intense fragrance. It spreads by rhizome, naturalizes under trees, and is hardy in Zones 2–9 — which means it persists in many gardens for decades. What most gardeners don’t know is that this plant contains at least 38 distinct cardenolide compounds, including convallarin and convallatoxin [3]. Not one glycoside — thirty-eight.

The ASPCA lists it as toxic to dogs, cats, and horses, with clinical signs including vomiting, irregular heartbeat, low blood pressure, disorientation, coma, and seizures [3]. Like oleander and foxglove, the toxic compounds persist in vase water — a pot of lily of the valley clippings on a low table is enough exposure risk if a cat or dog drinks from it.

One underappreciated feature of lily of the valley is its red berries, which ripen in late summer and are attractive to dogs. The berries contain the same cardenolide complex as the rest of the plant. A dog grazing through a naturalized patch in August may ingest berries without the owner connecting the fruit to the decorative spring-blooming ground cover they’ve had for years. If you have pets and lily of the valley, the entire planting — not just the spring flowers — needs to be managed or removed.

Phase 1 — Gastrointestinal (0–2 hours): Drooling, nausea, vomiting, diarrhea. Many owners interpret this as a mild stomach upset and wait to see if the pet improves. This is the phase where treatment is most effective — decontamination is still possible.

Phase 2 — Cardiac (2–6 hours, potentially persisting 4–5 days): Weakness, slowed or irregular pulse, difficulty walking, low blood pressure, collapse. The 2.4 kg Maltese case above entered this phase at 24 hours with a heart rate of 80 bpm and AV block [7]. A pet who “seems fine” after GI signs may be hours away from an arrhythmia.

The five symptoms that always warrant an immediate call to poison control or emergency vet:

  1. Any vomiting after known or suspected exposure to these plants
  2. Lethargy or weakness beyond normal tiredness
  3. Abnormal heart rate (too slow, too fast, or irregular rhythm you can feel)
  4. Difficulty walking, stumbling, or hind-limb weakness
  5. Pale or white gums (sign of poor cardiac output)

What the Vet Does: Treatment Steps

Knowing the treatment protocol helps you communicate clearly at the emergency vet and understand why certain steps matter.

Step 1 — Decontamination (if within 1–2 hours, pet not already showing cardiac signs): Induced vomiting removes remaining plant material from the stomach. Activated charcoal with a cathartic is given to absorb glycosides in the GI tract. Because cardiac glycosides undergo enterohepatic recirculation — they cycle from the intestine back into the bloodstream via bile — activated charcoal may be repeated every 6–8 hours for three total doses [6].

Step 2 — Monitoring: ECG to track heart rhythm, blood pressure monitoring, and electrolyte panels. Hyperkalemia (high potassium) is the critical prognostic marker: serum potassium above 5.5 mEq/L has been associated with poor outcomes in cardiac glycoside poisoning cases [4]. Intravenous fluids support blood pressure and help correct electrolyte imbalance.

Step 3 — Arrhythmia management: Atropine is used for bradycardia (heart rate below 40–50 bpm). Lidocaine or procainamide for ventricular arrhythmias [6]. In the documented cat case, a ventricular arrhythmia with no discernable P waves resolved within 15 minutes of Digibind infusion [5].

Step 4 — Digoxin-specific antibody fragments (Fab / Digibind): The specific antidote. It works by binding the cardiac glycoside molecules directly and neutralizing them for urinary excretion. Originally developed for digoxin overdose in humans, it also works for plant glycosides — oleandrin, digitoxin, and convallarin all respond to Fab treatment [4, 5, 8]. The cost (approximately $3,200 per vial) means it’s reserved for severe cases, but it can be lifesaving when arrhythmias don’t respond to standard antiarrhythmics [5].

Prognosis depends heavily on the amount ingested, how quickly the pet received care, and whether cardiac signs had already developed. With no cardiovascular abnormalities: excellent. With severe arrhythmias and no access to Fab: guarded to poor. Fatalities have been reported for both oleander and foxglove even with aggressive treatment [6].

ASPCA-Verified Safe Replacements

Removing these plants doesn’t mean giving up the garden styles they’re typically used for. Each has a safe substitute that fills the same design role.

Replace oleander (warm-climate flowering hedge, Zones 8–11):
Crepe myrtle (Lagerstroemia indica, Zones 7–10) is ASPCA-confirmed non-toxic to dogs and cats. It grows as a multi-trunk shrub or small tree, blooms summer through fall in pink, white, red, or purple, and handles drought and heat just as well as oleander. Replace foxglove (tall flowering spikes, Zones 4–8):
Veronica spicata (spiked speedwell, Zones 3–8) produces upright flower spikes in blue, purple, pink, or white through summer and is ASPCA-confirmed non-toxic to dogs and cats. It grows 12–18 inches tall, attracts pollinators, and doesn’t self-seed aggressively. Replace lily of the valley (shade ground cover, Zones 2–9):
Sweet alyssum (Alyssum spp., annual in most zones) is ASPCA-confirmed non-toxic to dogs, cats, and horses [11]. It produces small, fragrant white flowers and spreads as a low mat. For a perennial shade ground cover, creeping phlox (Phlox subulata, Zones 3–9) is ASPCA non-toxic and blooms in early spring with a similar low-growing, spreading habit. Removal Before Replacement

Safe plant removal is a multi-step process. Oleander in particular regrows vigorously from cut stumps and root fragments — surface removal is rarely permanent.
